Your primary care physician is the first person you turn to for your health care, so you need someone who's highly skilled, close by and reliable. Jefferson has more than 200 primary care experts. Many are conveniently located near your home or workplace.

Our primary care physicians:

  • Provide preventive care and immunizations
  • Perform comprehensive physical exams
  • Diagnose and treat common medical conditions, such as high blood pressure, diabetes and urinary tract infections
  • Teach healthy lifestyle behaviors to help with things like weight loss, smoking cessation and stress management
  • Coordinate with other specialists when necessary

With internal and family medicine offices located in Center City Philadelphia, Chinatown, the Art Museum area, South Philadelphia, the Navy Yard, along the Main Line, and South Jersey, our more than 200 primary care experts are just a short walk or drive from most of the Delaware Valley.

Continuing Care Program

Primary Care for Patients with Intellectual & Developmental Disabilities

Adolescents often face significant challenges finding the right fit as they transition to adult providers, especially those with intellectual and developmental disabilities or other complex childhood-onset conditions. Jefferson’s Continuing Care Program at the Jefferson Health – Navy Yard location, provides specialized care to help guide them and their care team so they can thrive in adulthood. Our experienced staff provides the highest quality primary care for patients with, but not limited to, the following conditions:

  • Autism
  • Cerebral Palsy
  • Cystic Fibrosis
  • Down syndrome
  • Spina Bifida

During COVID-19 (Coronavirus) Outbreak

We are still scheduling appointments for new and existing patients; however, we have made several temporary adjustments to how we deliver care to reduce your risk and ensure safety. Adjustments we’ve made include:

  • Video/Telehealth visits
    These visits allow you to use a mobile device (smartphone or tablet) or desktop computer with a camera to speak with your physician through a video connection. We continue to encourage you to use telehealth visits, which allow us to provide many needed healthcare services while you are in the comfort of your own home.
  • Expanded In-person appointments
    In-person visits, including early morning, evening and Saturday appointments, are available to patients with health concerns that require special accommodations or as preferred. We’ve developed a direct rooming policy to minimize time spent in waiting rooms and ensure all patients and care team members wear masks and appropriate personal protective equipment (PPE).
  • Increased Safety Measures
    Learn some of the necessary steps we’ve taken to ensure that all of our patients and healthcare providers are safe at all of our locations.
