Jefferson has long been a leader in geriatric medicine, which specifically addresses the unique care needs of older adults. Our doctors are both family physicians and geriatricians who see patients 55 years old and up.

Our Geriatric Medicine leadership has also been recognized by the American Geriatrics Society (AGS) and named president of the Association of Directors of Geriatric Academic Programs (ADGAP).

Special Programs for Older Adults

Geriatric Assessment Program

On the first, second, fourth and fifth Monday afternoons of the month, we see patients for a comprehensive evaluation that includes addressing issues like memory loss, dementia or other complex issues. Patients should be prepared for a 3-hour appointment. Please bring all medications and a family member or close friend.

Mini-Geriatric Assessment Program

On Wednesday afternoons, we conduct this mini assessment that lasts 1 hour and is meant to focus on one to two specific concerns.

Falls Prevention Program

On the third Monday afternoon of the month, fall assessments are available for a comprehensive evaluation of an older adult who has fallen or is at high risk for falling. This appointment lasts for approximately 3 hours.

Healthy Aging Program

Join the Jefferson Division of Geriatric Medicine and Palliative Care for a free workshop series for older adults.

Age-Friendly Health Systems

Jefferson Health is participating in Age-Friendly Health Systems, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) in partnership with the American Hospital Association? (AHA) and the Catholic Health Association of the United States (CHA). As part of the initiative, Jefferson Health is implementing a set of evidence-based interventions designed to improve care for older patients known as the "4Ms" – What Matters, Medication, Mentation and Mobility. Jefferson Health now joins more than 100 health systems working to make care for older adults even more tailored to patients’ goals and preferences and consistently of high-quality.
