Lose Weight and Prevent Diabetes - Diabetes Prevention Program

Presented by the Frazier Coalition for Stroke Education Webinar Series

If you are at riskfor diabetes, getting ahead of the curve and learning about prevention can help you reach your health goals. A trained lifestyle coach will help you lose weight, become more active and reduce stress to PREVENT TYPE 2 DIABETES.

The year long program consists of 16 weekly sessions for the first 6 months followed by 2 sessions per month in the second 6 months.

You can join the program if you are 18 years or older and have one of more of the following:

  • Overweight (BMI greater than Or equal to 25)
  • Have an HbA1C of 5.7% - 6.4% or have been diagnosed with prediabetes
