Hypertension Self-Management Education and Support Program

Presented by the Frazier Coalition for Stroke Education Webinar Series

To help you reduce your risk and avoid other complications, FFC offers a 6-month hypertension education and support program that will assist you in lowering your blood pressure.

We invite you to join this program if you are 18 years or older and have been told by your health care provider that you have hypertension. This program includes two visits with a registered dietitian and a free produce box from Farm to Family for 12 weeks.
