Community Diabetes Prevention Program

Jefferson’s Center for Urban Health has trained Lifestyle Coaches that can help you lose weight, become more active, and reduce stress. Overall prevent Type 2 Diabetes.

The Community Diabetes Prevention Program is a year-long program with weekly meetings for the first 6 months, then once or twice a month for the second 6 months.

You can participate in this free program if you are 18 years or older, overweight (BMI greater than or equal to 24), HbA1C of 5.7%-6.4, or have been diagnosed with prediabetes.
