Community Diabetes Prevention Program

If you're at risk for diabetes, getting ahead of the curve and learning about prevention can help you reach your health goals. 

At the Community Diabetes Prevention Program, trained lifestyle coaches from Jefferson’s Center for Urban Health are on hand to help guide you to lose weight, become more active, reduce stress and prevent type 2 diabetes.

The Community Diabetes Prevention Program consists of 28, one-hour sessions delivered over the course of a year.

You can participate in this free program if you are 18 years or older, overweight (BMI greater than or equal to 24), HbA1C of 5.7-6.4%, or have been diagnosed with prediabetes.

COST: FREE
