Lose Weight and Prevent Diabetes is the Jefferson Center for Urban Health’s Diabetes Prevention Program. Trained lifestyle coaches will help guide you to lose weight, become more active, reduce stress and prevent type 2 diabetes.
The online program consists of 28, one-hour sessions delivered over the course of one year.
You can participate in this free program if you are 18 years or older, overweight (BMI greater than or equal to 25), HbA1C of 5.7-6.4%, or have been diagnosed with prediabetes
