Lose Weight & Prevent Diabetes - Online
If you're at risk for diabetes, getting ahead of the curve and learning about prevention can help you reach your health goals.
This diabetes prevention program is presented by Champions of Hope: Latinos Preventing Diabetes. Trained lifestyle coaches will help guide you to lose weight, become more active, reduce stress and prevent type 2 diabetes.
The online program consists of 28, one-hour sessions delivered over the course of one year.
You can participate in this free program if you are 18 years or older, overweight (BMI greater than or equal to 25), HbA1C of 5.7-6.4%, or have been diagnosed with prediabetes

February 2022 – July 2022 = 12 sessions
