The Center for Urban Health works collaboratively with the Jefferson Diabetes Center to provide individuals and families living with diabetes and prediabetes education tools and resources to better self-manage diabetes, reduce complications and improve quality of life.

This four-session class takes place every Monday evening from April 30 through May 21.
