Community Diabetes Prevention Program
One out of three American adults has prediabetes, and most of them do not know it. Prediabetes means your blood sugar level is higher than normal, but not high enough to be diagnosed as diabetes. This raises your risk for Type 2 Diabetes, heart disease and stroke.
Proven to prevent or delay type 2 diabetes, Center for Urban Health Community Diabetes Prevention Program includes a:
- year-long program with weekly meetings for the first 6 months, then once or twice a month for the second 6 months;
- support from others like you as you learn new skills; and a
- curriculum approved by the Centers for Disease Control and Prevention.
You can participate in this free program if you are 18 years or older, overweight (BMI greater than or equal to 24), HbA1C of 5.7% or higher or have been diagnosed with prediabetes.
